Fellowship is the highest level of ACLM membership. Review the Membership Requirements here.

Note: Fellowship candidates initially join the College as an Associate members and work towards completing Fellowship requirements. If you are nearing completion of the requirements, you may choose to apply directly for Fellowship.

Before you begin:

1. Download the Fellow application form here and complete it: FELLOW APPLICATION FORM

2. Gather all required supporting documentation:

• Recent photograph of yourself
• CERTIFIED COPY of ID (1 item) either Passport, Driver’s License or Birth Certificate
• Digital copy of your current certificate of medical/dental registration
• CERTIFIED COPIES of medical/dental degree and transcripts
• CERTIFIED COPIES of any post graduate medical or dental training qualifications
• CERTIFIED COPIES of legal or forensic training degree and transcripts
• Current and complete Curriculum Vitae (include all positions held since graduation and any publications)
• Evidence of any awards
