If you are a medical practitioner or dentist with legal or forensic qualifications or working within that field, please apply for Associate membership instead. Review the Membership Requirements here.

Before you begin:

1. Download the Medical/Dental Affiliate application form here and complete it: MED/DENT AFFILIATE APPLICATION FORM

2. Gather all required supporting documentation:

• Recent photograph of yourself 
• CERTIFIED COPY of ID (1 item) either Passport, Driver's License or Birth Certificate 
• Evidence of your current healthcare or legal registration 
• CERTIFIED COPIES of your healthcare and/or legal/forensic degrees
• Evidence of ACLM course or event attendance
