2. Gather all required supporting documentation:

• Recent photograph of yourself 
• CERTIFIED COPY of ID (1 item) either Passport, Driver's License or Birth Certificate 
• Evidence of your current medical or dental university program enrollment
• CERTIFIED COPIES of completed relevant degree/s (if any)
• Evidence of any current registration (if any)
